Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fire and thermal burn hazards from the operation of a
Class IV carbon dioxide
laser.
In the Laser Area, employees operating the Mitsubishi LC10BV Class IV
carbon dioxide LASER
were:
a) unfamiliar with the hazards or the safety precautions while working
with the machine;
b) permitted to use a flammable piece of cardboard to shield the refracted
or reflected beam
Among other methods, one feasible and acceptable means of abatement to
correct this hazard
is to establish a laser safety program in accordance with American
National Standards Institute
(ANSI)  Z136.1-2007 American National Standard for the Safe Use Of Lasers,
and
the
manufacturer's instructions, including but not limited to:(1) Appointment
of a qualified Laser
Safety Officer; (2) Conduct of a hazard safety analysis and establishment
of safety-related
standard operating procedures; and, (3) Provision of safety training to
operators and other
affected employees.
