"Raymond"
stand up order
pickers were not correctly connected to the attachment point as they were
wrapped around the
attachment bar and connected back to themselves allowing the possibility
of damage and slip
out.
Abatement note: One means of abatement would be to acquire sized lanyards
that will not
allow more than 2 feet of fall with locking snap hooks that will connect
to the lifts anchor
point and the "D" ring on the  employees's restraint belt.
