North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fire and explosion hazards from Hydrogen gas: a)facility - where the employer did not perform maintenance on a hydrogen gas system used by employees to calibrate equipment exposing employees to fire and explosion hazards. ABATEMENT NOTE: One feasible abatement method, among others, to correct this hazard are, but not limited to: 1.Establish and implement a maintenance program to be performed annually. 2.The maintenance shall include inspection for physical damage, leak tightness, ground system integrity, vent system operation, equipment identification, warning signs, operator information and training records, scheduled maintenance and retest records, alarm operation, and other safety related features. 3.Scheduled maintenance and retest activities shall be formally documented, and records shall be maintained a minimum of 3 years. ABATEMENT NOTE: One feasible abatement, among others, to correct this hazard are, but not limited: 1.Install quick response (QR) fire sprinklers in accordance with national Fire Protection Act (NFPA) 13, Standard for the Installation of Sprinkler Systems Reference: National Fire Protection Agency (NFPA) 2, Hydrogen technologies Code, 2011 edition. Chapter 16, paragraph 16.2.3.1.1.1.

ABATEMENT NOTE: One feasible abatement method, among others, to correct this hazard are, but not limited to: 1.Installation of explosion-resistant walls or barricades around the laboratory work area containing the explosion hazard. 2.Installation of sufficient deflagration venting in outside walls to maintain the integrity of the walls separating the hazardous laboratory work area or laboratory unit from adjoining areas. Reference: National Fire Protection Agency (NFPA) 2, Hydrogen technologies Code, 2011 edition. Chapter 16, Explosion Hazard Protection, paragraph 16.2.4.2.1.

ABATEMENT NOTE: One feasible abatement method, among others, to correct this hazard are, but not limited to: 1.Install a laboratory ventilation exhaust system to include fume hoods to exhaust Hydrogen gas from the laboratory work areas where Hydrogen gas could or is released.
