North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to ammonia refrigerant: a)facility, ammonia room- where the employer did not post emergency shutdown procedures for maintenance employees who conducted routine inspections on refrigeration systems containing ammonia, and are subject to an uncontrolled release of this chemical. One feasible and acceptable abatement method, among others, to correct this hazard is the following: 1.Emergency shutdown procedures, including precautions to be observed in case of a breakdown or leak, shall be displayed on a conspicuous card located as near as possible to the refrigerant compressor. These precautions shall address: a)instructions for shutting down the system in case of emergency; b)the name, address, and day and night telephone numbers for obtaining service; and c)the names, addresses, and telephone numbers of all corporate, local, state, and federal agencies to be contacted as required in the event of a reportable incident. Reference: ANSI/ASHRAE Standard 15-2004, Safety Standard for Refrigeration Systems, Section 11.7 b)facility, ammonia room - where the employer did not post at the entrance to the ammonia room a sign prohibiting access by unauthorized personnel. One feasible and acceptable abatement, among others, to correct this hazard is: 1.At the entrance to each refrigerating machinery room, provide a legible permanent sign, securely attached and easily accessible, reading "Machinery Room - Authorized Personnel Only." The sign shall further communicate that entry is forbidden except by those personnel trained in the emergency procedures. Reference: ANSI/ASHRAE Standard 15-2004, Safety Standard for Refrigeration Systems, Section 11.2.4.
