29 CFR 1910.120(q)(1): The employer did not develop and implement a written emergency response plan to handle anticipated emergencies prior to the commencement of emergency response operations, involving uncontrolled anhydrous ammonia leaks, that was available for inspection and copying by employees, their representatives and OSHA personnel: a)Roof of the Debone plant: On or about June 4, 2011, the employer failed to develop and implement a written emergency response plan in accordance with 29 CFR 1910.120(q)(2) for maintenance personnel required to respond and control anhydrous ammonia releases. Refrigeration technicians responded to a rooftop anhydrous ammonia leak with an unknown atmospheric concentration, potentially IDLH, without an established plan and the availability and use of self-contained breathing apparatus.
