employer did not
furnish to each of his employees a place of employment which were free
from recognized
hazards that were causing or likely to cause death or serious physical
harm to his employees in
that employees were exposed to struck-by and crushed-by hazards:
a)On or about 5/13/2011 - Plant area, the Toyota forklift, Serial Number
75289,
was operated without the seat belt being accessible and tied up in a chain.
b)On or about 5/13/2011 - Plant area, the Toyota forklift, Serial Number
75501,
was operated with a missing seat belt.
Among other methods, one feasible and acceptable method to correct this
hazard is to ensure that
seat belts are provided and utilized, as stated in ASME B56.1b-2003,
Safety Standard for Low
Lift and High Lift Trucks, Section 7.41, Operator Restraint Systems.
