Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls: a) Warehouse Self-retracting lanyards did not retract exposing employees to fall hazards. Violation observed on or about 05/06/2011. b) Warehouse Damaged harness used as fall protection exposing employees to fall hazards. Violation observed on or about 05/06/2011. Among methods, all of the following are feasible and acceptable means of abatement: Replace any body belt, lanyard, or deceleration device that has sustained permanent deformation or otherwise damaged in accordance with ANSI/ITSDF B56.1-2009 Safety Standard for Low Lift and High Lift Trucks, Section 4.17.2. c) Warehouse Lanyard attached to side D-rings on harness exposing employees to fall hazards. Violation disclosed on or about 05/06/2011. Among methods, all of the following are feasible and acceptable mean of abatement: Connect lanyard in accordance with the manufacturer's Instructions for Use, Installation & Operation Section.
