were exposed to amputation and crushing hazards:
a.On or about November 9, 2011, the employer did not ensure the Bobcat skid
steer loader located and operated in the fertilizer blending plant was
equipped
with a complete bobcat interlock control system (BICS) seat/safety bar that
prohibited employees from access and egress without being raised which
deactivates the lift, tilt, auxillary hydraulics, and traction controls as
required by
the Bobcat manufacturers S650 skid steer loader operation & maintenance
manual
and the safety manual for operating and maintenance personnel, exposing
employees to amputation and crushing hazards if the section of the bar to
the left
of the operator was not raised before egress from the loader.
Feasible and acceptable methods to correct the hazard include, but are not
limited
to the following:
Obtain and install the manufacturers complete seat/safety bar for the
Bobcat
model: S650 Skid Steer loader.
ABATEMENT CERTIFICATION AND DOCUMENTATION ARE
REQUIRED.

Obtain and install approved manufacturers safety restraint device seat
belt and
driver side door for the Mack model 89R yard truck.
