Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
a.On or about March 20, 2012, the employer did not require employees to
perform
a daily check of the equipment as required by the Operator's Manual and
ANSI
A92.5.
A fessible means of correction would be to institute a policy for
inspecting
equipment prior to use.  The inspection should include:  (1) All functions
and
their controls for speed(s), smoothness, and limits of motion; (2) Lower
controls
including the provisions of overriding of upper controls; (3) All chain
and cable
mechanisms for adustment and worn or damaged parts; (4) All emergency and
safety devices; (5) Lubrication of all moving parts, inspection of filter
element(s),
hydraulic oil, engine oil, and coolant as specified by the manufacturer;
(6) Visual
inspection of structural ccomponents and other critical components such as
fasteners, pins, shafts, turntable attachment bolts, and locking devices;
(7)
Placard, warnings, and control markings; (8) Additional items specified by
the
manufacturer; (9) Correction of all malfunctions and problems identified
and
further inspection, if necessary; and (10) After correction or replacement
of any
component, the unit shall be inspected and tested again.

REQUIRED.TION
