over the Buffalo River, Buffalo, NY: Two employees conducting an annual
bridge
inspection were exposed to a fall of approx. 17 ft. 4 in. into the pit
through triangular
floor holes located in the Northeast and the Northwest corners of the
counterweight
chamber. One employee was injured when he fell through the hole in the
northeast
corner. The holes were not protected by standard railings or by floor hole
covers.
