North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical shock, burns, and/or minor respiratory irritation from smoke inhalation from unused openings in electrical outlets and panels, junction boxes without covers, flexible cords with spliced wiring and strain relief failure. a) Facility, Layer House No. 4 - Dayton AC Gear Motor, mounted on a metal cage row's de-escalator, 115 vac, hp, with an unused opening where the power cord enters the motor. The opening measured approximately 1-inches by 3-inches. b)Facility, Pullet House No. 2. Federal Pacific circuit breaker box with 14 unused openings for screw-in type plug fuses. c)Facility, Pump House No. 1. Square D Company circuit breaker panel with 2 unused openings; 3-inches by 2 -inches and 1-inch by 2 -inches. d)Facility, Pump House No. 3. A junction box, 120 vac. without a cover. The junction box measured 4- X 4-inches and was at floor level. e)Facility, Layer House No. 6, lower level near entrance. A metal junction box, 120 vac. without a cover. The box measured 4- X 4-inches. f)Facility, Layer House No.6. A black-colored flexible power cord had been spliced. The spliced areas were covered with grey tape. g)Facility, Layer House No. 10. Air compressor's power cord had been sliced in 2 locations. One splice was covered with red tape, a 2nd connected with wire nuts. The spliced power cord was laying on the compressor's metal tank. h)Facility, Pump House No. 2. Wiring for a water pump had been spliced. The spliced wiring was connected with wire nuts and also covered with black tape. i)Facility, Processing Plant, Quality Control Room - The flexible power cord for the Feather Lite light had strain relief failure. Abatement Note: One feasible and acceptable means of abatement is to hire a licensed electrical contractor to repair this equipment in accordance with the National Electrical Code and local codes.

Fall exposure of 7-feet, 8-inches to a lower level. b)Facility, Pullet House No. 3 - Missing midrail on platform at front entrance. Fall exposure of 8-feet, 5-inches to a lower level. c)Facility, Layer House No. 1 1. No midrails on stairway to front entrance. 2. No midrails on 3 sides of platform at front entrance. 3. One open side on platform at front entrance. Fall exposure of 9-feet to a lower level d)Facility, Layer House No. 9 - Platform at rear entrance had 2 open sides. The fall exposure to a lower level was 8-feet, 10-inches. e)Facility, Layer House No. 10 1. Platform at front entrance has 3 open sides. The fall exposure to a lower level was 8-feet, 3-inches. 2. Platform at rear entrance had 1 open side. The fall exposure to a lower level was 8-feet, 6-inches. Abatement Note: One feasible and acceptable means of abatement is to provide standard railings on stairways, platforms, and floor openings.
