Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to employees employment and a place of employment which were free
from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees. Employees were exposed to the hazard of being crushed by a
moving hoist car
while working underneath the car.
a)South side of building:
Employees were exposed to being caught and crushed between the bottom of
the hoist car and the hoist support column while removing bottom terminal
limits during the dismantling of the exterior hoist system. Safety
procedures
were not taken. On or about 04/20/11.
Among other methods, one feasible and acceptable method to correct this
hazard is:
Comply with the American National Standard A10.44-2006 Control of Energy
Sources (Lockout/Tagout) for Construction and Demolition Operations, and
while working underneath the car.

hazard is:
Comply with the American National Standard A10.44-2006 Control of Energy
Sources (Lockout/Tagout) for Construction and Demolition Operations, and
establish and enforce a program consisting of energy control procedures,
employee training, and periodic inspections to ensure that before any
employee
performs any servicing or maintenance on a machine or equipment where
unexpected energizing, startup or release of stored energy could occur and
cause injury, the machine or equipment shall be isolated from the energy
source and rendered inoperative.

OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT,
OR ESSENTIALLY SIMILAR STANDARD, Section 5(a)(1) of the Occupational
Safety and Health Act of 1970, WHICH WAS CONTAINED IN OSHA
INSPECTION NUMBER 312247612, CITATION NUMBER 01, ITEM NUMBER
001, ISSUED ON 10/31/2008 WITH A FINAL ORDER DATE OF 04/06/2009.
NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE
EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF
ABATEMENT FOR THIS ITEM.  FAILURE TO COMPLY MAY RESULT IN
AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
