On October 25, 2011, Mangual Office Cleaning Services, Inc. failed when
transported employee
in the back of a Ford 150 pickup without any protection that would prevent
employees from
falling of the vehicle. In addition and worsening the situation it was
documented that  materials
such as trimmers and containers of gasoline were also transported ont the
back
of the pickup
A method for correcting the hazard: Transporting employees in a larger
vehicle that contains
chairs with belts.
Hazard: That employees fall from the vehicle while being transported.
El 25 de octubre de 2011, Mangual Office Cleaning Services Inc, fallo
cuando transporto a
empleados en la cabina trasera de una guagua, marca Ford 150 sin ningun
tipo de proteccion.
Se documento que empleados eran trasportados junto a materiales de trabajo
como trimers y
contenedores de gasolina sin tapa por via de rodaje principales.
Un metodo para corregir el riesgo: Trasportar empleados en un vehiculo mas
grande que
contenga sillones con cinturones.
Riesgo: Que empleados caigan del vehiculo mientras son transportados

On October 25, 2011 on Route 66 in Carolina Puerto Rico, Mangual Office
Cleaning Services
failed when not maintained the gasoline containers kept covered to prevent
leakage. It was
observed and documented that employees used two plastic containers of
gasoline of 10 gallons
each without cover for supplying gasoline to the trimers and the gallons
remained in the rear
cabin of the vehicle remained even when in motion.
Method to abate the hazard: Keep gasoline containers covered at all times
to prevent leakage or
fire ignition source.

El 25 de octubre de 2011 en la Ruta 66 en Carolina Puerto Rico, Mangual
Office Cleaning
Services fallo cuando no mantuvo los contendores de gasolina tapados para
evitar derrames. Se
observo y se documento que empleados utilizaban dos contendores plasticos
de gasolina de 10
galones cada uno para suministrar gasolina a los trimers y que los
recipientes permanecian en
la cabina trasera del vehiculo sin tapa aun cuando este permanecia en
movimiento.
Un metodo para corregir el riesgo: Mantener tapados los contenedores de
gasolina en todo
momento para evitar derrames o incendios por alguna fuente de ignicion.

causando o que puedan
causar muerte o dano fisico a los empleados, por cuanto empleados estaban
expuestos a:
Mangual Office Cleaning Services failed to provided protecting equipment
as warning signs and
traffic control devices for employees who perform work in the road. It was
observed and
documented employees working with trimmers cutting grass on Route 66 in
the direction of
Carolina without signs that indicates to drivers precautions with
employees on the roads.
Method of correcting for Hazard: The employer shall follow the
instructions
and specifications
of the Manual of Uniform Control Devices Road Traffic and Highways
Department of
Transportation United States.
Hazard: Employees are hit by cars
Mangual Office Cleaning Services fallo cuando no proveyo equipo como
letreros de avisos y
dispositivos de control de transito para empleados que realizaban trabajos
en la carretera. Se
observo y se documento que empleados trabajaban cortando grama con trimers
en la ruta 66 en
direccion hacia Carolina sin proteccion de avisos que indicaran a los
conductores que empleados
trabajan en las vias de transito.
Metodo para corregir el riesgo: El patrono debera seguir las instrucciones
y especificaciones del
Manual de Dispositivos de Control Uniforme de Transito en Calle y
Autopistas del
Departamento  de Transporte de los Estados Unidos.
Riesgo: Empleados sean golpeados por automoviles.
