causing or likely to cause death or serious physical harm in that;
employees using portable
gasoline powered cut off/chop saws were exposed to struck by and/or
laceration hazards from
saw kickbacks while cutting inadequately supported materials: (a)Roadway
and excavation
construction site located at the intersection of Appleton Street and
Crescent Street in Holyoke,
Massachusetts; On August 12, 2011, the employer did not ensure that
concrete pipes with belled
ends were adequately supported to prevent saw kickbacks during cutting
activities. An employee
was injured from a saw kickback while using a STIHL TS420 cut off/chop saw
equipped with
a diamond blade to cut concrete piping for installation of new manholes.
AMONG OTHER
METHODS, FEASIBLE AND ACCEPTABLE MEANS OF HAZARD ABATEMENT include:
*Develop and implement a comprehensive training program which specifically
addresses
appropriate means of supporting materials based on the type of work
activities being performed
and/or material being cut. *Follow recommendations from STIHL's
Cut-off-Machine
Safety
Manual and STIHL's TS420 Owners Manual, which both identify that materials
being cut must
be fully supported in such a way that the cut will remain open during the
cutting process and
upon completion of the cut.
