General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to lacerations from the Bar Maid Glass Cleaning Machine in the bar area: On or about 9/20/11 and for thirty days prior, employees were exposed to lacerations from broken glass when all four outside brushes were not securely in place before operating the Bar Maid A-200 glass washer and the center brush was not appropriate for the size of the glass being washed. On 8/22/11 an employee received severe hand lacerations from broken glass while using the glass washing machine in the aforementioned configuration. Feasible and acceptable means of abatement includes, but is not limited to, following the operating instructions provided by the manufacturer and ensuring the appropriate attachments are securely in place prior to operation.
