Health Act of North
Carolina:  The employer did not furnish employment and a place of
employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that employees were exposed to fire and explosion hazards:
a)USF-44 dust collector, wheelabrator drum blaster area, employees working
in
the
shot blast area were exposed to fire and explosion hazards in that the
USF-44
Cartridge Dust Collector for Amasteel shot blast was located inside of the
building.  (Reference NFPA 484, 12.2.1.4)
b)USF-44, wheelabrator drum blaster and lid blaster areas, employees
working the
shot blast areas were exposed to fire and explosion hazards in that
Amasteel shot
abrasive blaster machines, duct work, and dust collection drums/lids were
not
bonded and grounded to control ignition hazards from static sparks.

c)facility, lid blaster dust collection system was located outside of the
building, but
there was no equipment in the ducting to vent or isolate a dust
deflagration.

d)combustible dust collection systems, a documented Risk Evaluation was not
conducted to determine the level of explosion protection to be provided
for a dust
collection system.  (Reference NFPA 484, 12.2.5.1); a process hazard
analysis
for the design of the fire and explosion safety provisions was not based
on a
process hazard analysis of the facility, the process, and the associated
fire or
explosion hazards.  (Reference NFPA 654, 4.2.1)
Among other methods, one feasible and acceptable method to abate these
hazards
is to comply with National Fire Protection Agency (NFPA) 484, "Standard for
Combustible Metals" (2009) and National Fire Protection Agency (NFPA) 654,
"Standard for the Prevention of Fires and Explosions from the
Manufacturing,
Processing and Handling of Combustible Particulate Solids" (2006).
Abatement Note: The Process Hazard Analysis must be documented and
reviewed/updated at least every 5 years.
