Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The Employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a)Elevator No. 6 (Device No. 1F 3081): The driving machine brake of a freight elevator was not maintained working order to prevent unplanned descent of the car. On or about 5/8/11. Abatement Note On feasible and useful method, among others, to eliminate or to materially reduce this hazard would be to comply with the provisions of American Society of Mechanical Engineers (ASME) A17.1-2000 Section 8.6.1.2 as referenced in ASME 17.1-2002 and maintain elevator equipment in working order.
