Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/03/11 Location: Acorn Trail Equipment:Altec Vehicle Mounted Aerial Device, Model #LRV-56, Serial #0302CV1398 Condition:An employee was exposed to fall hazards as great as 25 feet while performing tree trimming activities from an aerial device without performing daily preoperational inspections. Among other methods, one feasible and acceptable abatement method to correct the hazard, is daily preoperational inspection as well as routine maintenance of the equipment in accordance with the current verison of the ALTEC, LRV Series, Operators Manual (2006), Section 4.3 Daily Preoperational Inspections. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
