Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to crushing injuries:
(a) Jobsite, 1316 Third Avenue, Arnold, PA 15068: On August 29, 2011,
employees were
setting 58' 2" wooden parallel chord trusses at a New L&L Roofing
Warehouse/Office Building.
The top chord of the roof trusses were braced approximately every 18'
instead of every 15'
beginning at the ends of the trusses, one nail was used to secure the
bracing to the top chord
instead of two nails, braces were butted together on the top chord instead
of overlapping two
trusses, and 27 sheets of 4'x 8' x 5/8" Square Edge OSB was set on the top
chord instead of a
maximum of 16" as required by the Wood Council of America and Truss Plate
Institute
guidelines outlined in Building Component Safety Information, BCSI- B2 and
BCSI- B7: Guide
for Handling, Installing, Restraining, and Bracing Trusses, dated 2006.
The parallel chord
trusses were not uniformly braced over their entire installed length.
These conditions caused the
parallel chord trusses to collapse injuring two workers.
Among other methods, one feasible and acceptable means of abatement is to
follow the
recommendation of the component manufacture, along with the Wood Truss
Council of America
and Truss Plate Institute guidelines outlined in Building Component Safety
Information, BCSI-
B1: Guide for Handling, Installing, Restraining, and Bracing of trusses,
the BCSI-B2: Truss
Installation and Temporary Restraint/Bracing, and the BCSI-B7: Temporary
and Permanent
Restraint/Bracing for Parallel Chord Trusses, dated 2006.
