recognized hazards that
were causing or likely to cause death or serious physical harm to

were exposed to:
a) Fabrication Department - A minimum safe distance was not developed or
implemented
for employees who used hydraulic brakes including, but not limited to an
Amada 100 ton
brake (model R6-100, serial number 105521) and an Amada 80 ton (model
R6-80, serial
number 810316). On or about 6/16/11.
A feasible means of abatement would be to establish and implement a
minimum safe
distance program that conforms to the requirements of ANSI B11.3-1982
paragraph
6.1.4.3 and contains the training elements listed in OSHA Directive CPL
2-1.25.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT
CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN
AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR
1903.19.
