furnish employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees:
(a)Date of Violation:04-14-11
Condition:The employer did not provide proper supervision
and training to employees using a personal fall
arrest system while working from a boatswain's
chair.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to ensure that employees using a manual
boatswain's chair are properly trained and supervised according to
ANSI/ASME A39.1-1991 3.3.
Abatement documentation (photographs, receipts, copies of
programs, training certification, etc.) is required for this
item.red

Condition:An employee worked from a manual boatswain's chair
suspended 100 feet above the ground.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to ensure that employees do not exceed 75 feet
above grade using a manual boatswain's chair as directed by the ANSI /
ASME A39.1-1991, Section 6.2.1.
Abatement documentation (photographs, receipts, copies of
programs, training certification, etc.) is required for this
item.red
