Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the tip over hazards of an Powered Industrial Truck:
a)  On or about 09/09/11, and at times prior there to, an employee was
exposed
to a tip-over /crush-by hazard while operating a Gradall, Model G6-42a,
forklift
and not wearing the required seatbelt.  The employee was using the
forklift to
move material around the staging area.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to follow ITSDF/ANSI B56.1-2005, Section 5.1.4 which requires that all
operators read and
follow the manufacturer's operator manual and abide by the safety rules
and practices.er's
