Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck-by falling equipment: On or about 10/23/11 Wire rope clips used to connect the guylines to the anchors were installed backwards. The saddle was on a "dead horse" or the dead non load side of the wire rope. One method of abatement among others is to use "fist" grip clamps that have a saddle on both sides and follow the guidelines of Section 9.6.3c of the American Petroleum Institute's Recommended Practice 54 (3rd Edition 1999), as well as Crosby clip manufacturer guidelines. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that wire rope fittings are installed using manufacturer instructions.
