Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer
does not furnish to each of his employees employment and a place of employment w
hich are free from recognized hazards of being struck by power industrial truck
which are causing or likely to cause death or serious physical harm to employees
: a) The employer does not train operators in the safe operation of powered indu
strial trucks. This violation was observed on or about 10/06/11, in the producti
on area, employees were observed operating powered industrial trucks/forklifts.
The employer did not ensure that employees operating powered industrial trucks u
sed/wore seatbelts, there by the employees were exposed to the hazards of fallin
g out of and/or being struck by the powered industrial truck. Among other feasib
le methods to correct the identified hazard, one methods would be to follow ITSD
F/ANSI B56.1-2005, Section 5.1.4, which requires that
all operators read and follow the manufacturer's operator manual and abide by th
e safety rules and practices. Pursuant to 29 C.F.R. 1903.19, within ten (10) cal
endar days of the date of this citation, the employer must submit documentation
describing the actions it is taking to establish, communicate, and implement wri
tten procedures to ensure that powered industrial truck operators wear the requi
red seatbelts.

aterials which are likely to cause death or serious physical harm to employees i
n that employees: a) The employer is allowing hoist pendants to be used without
marking indicating the direction of the resultant motion. This violation was obs
erved on or about
11/08/11. Employees operating a Air Trac, 1 ton crane, with a Dayton 1 ton hoist
were exposed to a struck by hazard. This crane is located in the corridor betwe
en the Bronze and Iron furnaces. The function of one of the buttons of the 8 but
ton hoist pendant was not labeled. Also another button of this pendent was missi
ng entirely. b) The employer is allowing hoist pendants to be used without marki
ng indicating the direction of the resultant motion. This violation was observed
on or about 11/08/11. Employees operating a 3 ton crane, with a 3 ton CM Loadst
ar hoist were exposed to a struck by hazard. The crane is located in front of th
e Iron furnace. The function of one of the buttons of the 4 button hoist pendant
was not labeled. c) The employer is allowing hoist pendants to be used without
marking indicating
the direction of the resultant motion. This violation was observed on or about 1
1/08/11. Employees operating a 1 ton crane, with a Dayton 1 ton hoist were expos
ed to a struck by hazard. This crane is located in front of the Bronze furnace.
The function of the 2 button hoist pendant was not labeled. Also the buttons wer
e not visible, due to the pendant being mounted facing down toward the ground. d
) The employer is allowing hoist to be used at the workplace without ensuring th
e hoist are periodically inspection. This violation was observed on or about 11/
08/11. Employees operating a Air Trac, 1 ton crane, with a Dayton 1 ton hoist we
re exposed to a struck by hazard. This crane is located in the corridor between
the Bronze and Iron furnaces. The employer was unable to provide inspection reco
rds documenting, the periodic inspection for this crane.
e) The employer is allowing hoist to be used at the workplace without ensuring t
he hoist are periodically inspection. This violation was observed on or about 11
/08/11. Employees operating a 3 ton crane, with a 3 ton CM Loadstar hoist were e
xposed to a struck by hazard. The crane is located in front of the Iron furnace.
The employer was unable to provide inspection records documenting, the periodic
inspection for this crane. f) The employer is allowing hoist to be used at the
workplace without ensuring the hoist are periodically inspection. This violation
was observed on or about11/08/11. Employees operating a 3 ton crane, with a Day
ton 1 ton hoist were exposed to a struck by hazard. The crane is located in fron
t of the Iron furnace. The employer was unable to provide inspection records doc
umenting, the periodic inspection for this crane.
g) The employer is allowing hoist to be used at the workplace without ensuring t
he hoist are periodically inspection. This violation was observed on or about 11
/08/11. Employees operating a 1 ton crane, with a Dayton 1 ton hoist were expose
d to a struck by hazard. This crane is located in front of the Bronze furnace. T
he employer was unable to provide inspection records documenting, the periodic i
nspection for this crane. Among other feasible methods to correct the identified
hazard, one method would be to ensure hoist pendant are mark per ANSI B30.16 Ov
erhead Hoists (Underhung), Paragraph 16-1.1.2. Ensure that a period inspection p
rogram is established per ANSI B30.16 Overhead Hoists (Underhung), Paragraph 16-
2.1.3. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date
of this citation, the employer must submit documentation showing that it is in c
ompliance
with the standard, including describing the steps that it is taking to ensure th
at hoist pendants buttons at the workplace are properly marked and remain so, th
e steps that it is taking to ensure hoist in the workplace are periodically insp
ected.
