Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of struck by:
a)  On or about 10/27/11, and at times prior thereto, an employee was
operating
a Skytrack forklift, model 8042.  The employee was using the forklift to
raise a
personnel platform that had a width greater than the overall width of the
truck
plus 250mm (9.8 in.) on either side.
Among other methods, one feasible and acceptable means of abatement would
be to ensure the
work platform is designed and constructed according to the acceptable
requirements of the
American National Standards Institute/Standards for Rough Terrain Forklift
Trucks, ANSI
B56.6-2002, Part III, Section 8.24.1, platforms for elevating personnel.
