Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazards of:
a)On or about 01/05/2012, and at times thereto, employee(s) were exposed to
the hazards of being struck-by or crushed, while working underneath a lift
and
the anchor bolts were not tighten to the manufactures recommendation of 35-
45 ft/lbs.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO FOLLOW MANUFACTURES
RECOMMENDATION TO INSPECT ALL ANCHOR BOLTS MONTHLY AND RE-
TIGHTEN IF NECESSARY. ANOTHER FEASIBLE ABATEMENT WOULD BE TO
FOLLOW AMERICAN NATIONAL STANDARDS FOR AUTOMOTIVE ANSI B153.1-
2009.
