Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: a) Storage Area, On or about 2/17/12 and at times prior thereto employee(s) were exposed to a fall hazard of approximately 10 1/2 ft. when using a work platform that had the following hazards: a top rail measuring approximately 30 inches, no mid-rail, no toe board, no method of attaching the platform to the forks of the forklift, and was not marked with the maximum capacity of the platform, while retrieving items from the storage area such as but not limited to press parts. AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE TO: USE A WORK PLATFORM THAT MEETS THE REQUIREMENTS OF AMERICAN NATIONAL STANDARDS INSTITUTE (ANSI/ITSDF) B56.1-2012 SECTION 7.37 PLATFORMS: ELEVATING SUBSECTION 7.37.1 AND SUBSECTION 7.37.3 WHICH REQUIRE BUT IS NOT LIMITED TO THE FOLLOWING: A GUARD RAIL AT A MINIMUM HEIGHT OF 915 MM (36 IN.) TO A MAXIMUM HEIGHT OF 1065 MM (42 IN.), AND INCLUDE A MID RAIL, A TOEBOARD WITH A MINIMUM HEIGHT OF 100 MM (4 IN.), A MEANS OF SECURELY ATTACHING THE PLATFORM TO THE LIFTING FORKS AND HAVE THE FOLLOWING INFORMATION INDICATED ON THE PLATFORM: MAXIMUM LOAD, WEIGHT OF EMPTY PLATFORM, AND MINIMUM CAPACITY OF TRUCK ON WHICH THE PLATFORM CAN BE USED.
