a) On or about 10/14/11 at 111 Cedar Grove Lane, Somerset, NJ 08873:
Employees
worked at heights of 6'8" from the ground below from a job-made elevated
work
platform that was resting on the forks of a Pettibone 8044 (Rough Terrain
Forklift
Truck).
The basket/platform was:
1) not approved.
2) missing guardrail system/fall protection.
3) not secured to the forks of the Pettibone 8044.
4) employees left elevated while Pettibone 8044 was unattended.
In addition, the employees working on the platform were not wearing a full
body harness
with shock absorbing lanyards. No fall protection was provided at 6'8"
from ground level.
ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD,
AMONG OTHER MEANS, IS TO FOLLOW THE GUIDANCE SET FORTH IN THE
PETTIBONE 8044 TRAVERSE LIFT OPERATOR AND SAFETY MANUAL, WHICH
PROHIBITS THE USE OF A NON-APPROVED HOME MADE WORK PLATFORM.
