Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to:
On or about July 21, 2011, employees were exposed to fall hazards where
the wire rope
used for the Geronimo line system had the wire rope clips attached with
U-bolts that
were not orientated properly. Employees were exposed to the hazards
associated with
being struck by and falls. Among others, one feasible and acceptable
method of
abatement would be to comply with API RP 9B "Recommended Practice for
Occupational Safety for Oil and Gas Well Drilling and Servicing
Operations" for the safe
manner to attach the wire rope clips.
Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the
abatement date, the
employer must submit documentation describing the actions it is taking to
establish a safe
method to correct the hazard.
