Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling and being crushed by a structural collapse of an underground sports court. a)Worksite, 19 Meadowood Ln, Brookville NY - Employees were working on and around an underground sports court. The roof of the court collapsed causing employees on site to fall into the hole; on or about 07/27/11. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: Follow the Steel Joist Institute (SJI) Load Table for Longspan Steel Joists, LH series. Adopted by the Steel Joist Institute May 25, 1983, Revised to November 10, 2003 - Effective March 01, 2005. Refer to the SJI Load Table for the allowable load placed on the 36LH12 steel joists spanning approximately 65ft. Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
