North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)within the facility, employees did not use safe procedures when servicing rim wheels; on June 22, 2011, an employee was fatally injured while attempting to mount a 14" tire onto a 14 1/2" rim wheel. Among other methods, one applicable and feasible method to correct this hazard is to train employees on the hazards involved in tire servicing and the safety procedures to follow while servicing rim wheels.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a)within the facility, the employer did not provide equipment to allow employees to mount tires safely; an employee was in the process of mounting a 14" diameter trailer tire onto a 14.5" diameter trailer rim when the brake rotor he was using as a restrainng device erupted, causing a fatal injury. Among other methods, one applicable and feasible method to correct this hazard is provide employees proper tire changing equipment such as a Hunter, Model # TCX550BPS tire changing machine, which would eliminate the employees utilizing a brake rotor as a restraining device on the older model tire changing machines .
