29 CFR 1910.23(b)(1):  Open-sided floors and/or platforms four feet or
more above adjacent
floor or ground level were not guarded with standard railings (or
equivalent) and toeboards:
a. At the facility located on 1600 Kathleen Blvd. in Lakeland (FL).
Employees are exposed to a 30-ft fall hazard to the ground when conducting
activities such as, but not limited to, mechanical integrity inspections to
components of the ammonia refrigeration system located on the roof area. As
observed on or about 06/08/2011, roof sections were missing guard rails,
such
as those installed at other locations.
