Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees, in that,
employees were exposed to a fall hazard while maintaining the Greenhouse
screens:
a. "C" Greenhouse, employees were exposed to a 12 feet fall hazard while
maintaining the screens. The employees were required to climb on top of the
wooden greenhouse frames and walk on the 2 by 4 or 4 by 4 beams while
nailing down any loose screening. Fall protection was not provided to
protect
employees in the event of a fall. The integrity and stability of the beams
was
not established to assure that they could support the employee's weight.
Among other methods one feasible and acceptable method to correct this
hazard is to prohibit employees from climbing on top of the wooden frames
and establish a procedure for securing the screening from below using
ladders
or other equipment such as aerial lifts, scissor lifts or scaffolding.

employees, in that,
employees were exposed to the hazard of being struck by fragments of
plastic pipe used to
transport compressed air for the operation of air operated tools and air
used for cleaning, in
the event the piping ruptures:
a. Shipping area, polyvinyl chloride ( PVC ) plastic pipe was used to
transport
compressed air in exposed above ground locations at 100 psi, for the
operation
of air operated tools and air used for cleaning, a practice prohibited by
the
Plastics Pipe Institute and by ANSI/ASME B31.8-1986, Gas Transmission and
Distribution Piping Systems standard.
Among other methods one feasible and acceptable method to correct this
hazard is to replace
the PVC piping with a type that is approved for transporting compressed
gases in exposed
above ground locations, such as iron or steel piping.
