recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fire and explosion hazards at the Sid Richardson
Carbon Black
Co., Borger, TX facility:
On or about February 29, 2012, while attempting to ignite reactor #5, an
employee
was was
exposed to explosion/flash fire hazards. The employer did not develop,
document, and utilize
written lighting procedures for reactors. The employer did not ensure that
employees utilized
personal protective equipment that protected them against the hazard of
flash fire.  The
equipment used to ignite the reactors does not contain a reliable ignition
source to insure safe
ignition and prevent the accumulation of hazardous concentrations of
unignited fuel in the
reactor.  Among other methods, one feasible and acceptable abatement
method to correct this
hazard is to establish and implement procedures that ensure that the
ignition source  be
applied at the design location with the designed intensity to ignite the
air-fuel mixture; to
ensure that employees performing this operation are adequately trained in
the procedure; and
that the reactor is equipped with a reliable ingition source be installed
to ensure ignition of
the main burner and methods to prevent the accumulation of hazardous
concentrations of
unignited fuel-air mixture when ignition fails, as outlined in the
National Fire Protection
Association NFPA 86, Standard for Ovens and Furnaces 1969 Edition.
