Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) Location - 145 Farnum Street, North Andover, MA: On or about 7/15/11, the employer failed to ensure that a worker in a tree topping position was adequately secured until the work was complete. Among other methods, one feasible and acceptable method to correct this hazard is to train and/or retain employees to properly tie safety knots and observe the securing of safety knots before commencement of work in accordance with ANSI Z133.1-2006 "Safety Requirements for Arboricultural Operations".
