recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of struck by:
On or about August 25, 2011, and times prior thereto, where base-mounted
pillar jib
cranes were used to lift the main shaft of helicopters, the employees were
using jib
cranes which had not been inspected, tested or properly labeled with
lifting capacities.
a) This violation most recently occurred at the Textool Company, LLC, in
Houston, Texas when the employer failed to conduct initial, frequent and
periodic inspections of the crane.
ABATEMENT NOTES: Among other methods, one feasible method of
abatement to reduce this hazard is to follow the requirements of the
American
Society of Mechanical Engineers ASME B30.16-1998, 16-2.1.1 Inspection
Classification Guidelines. These include an initial Inspection. Prior to
initial
use, all new, altered, or modified hoists shall be inspected by a
designated
person to verify compliance with the applicable provisions of this volume.
b)The employer failed to maintain records of inspection of the jib crane
and
hoist; This  violation most recently occurred at the Textool Company, LLC,
on November 17-18, 2011, and times thereto, when the employer failed to
maintain records of inspections.
ABATEMENT NOTES: Among other methods, on feasible method of
abatement to reduce this hazard is to follow the requirements of the
American
Society of Mechanical Engineers ASME B30.16-1998 Guidance document.
Section 16-2.1.5 addresses Inspection Records and states that dated
inspection
reports and records should be maintained at time intervals specified in
Paragraph 16-2.1.2(b)(2). Records should be stored where they are
available to
appointed persons.
