Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: an electrical hazard of electrocution.
On or about November 10, 2011 in Dismantling and the Cut Pad:
Employees required to use six Rotary Automotive Lifts and one Grand
Automotive Lift
were exposed to struck-by hazards when the employer failed to take the
lifts out of
service after being notified by mechanics that the lifts were unsafe to
use.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to require and ensure that all vehicle lifts are properly maintained and
repaired prior to use as
required by ANSI/ALI ALOIM-2000: 6.2.1  whenever repairs and/or
maintenance
procedures
are called for by the lift operator, qualified lift inspector or
preventive maintenance technician.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
