Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed by a falling/collapsing vehicle while working beneath a malfunctioning and inappropriately modified automotive lift: a) Wentz Auto Body, Shop B: Employees were exposed to a crush hazard while working beneath vehicles elevated by an in-ground automotive lift and the employer failed to ensure that the lift's mechanical locking device was properly functioning, on or about 03/21/2012. b) Wentz Auto Body, Shop B: Employees were exposed to a crush hazard while working beneath vehicles elevated by an in-ground automotive lift and appropriately engineered height extension adapters were not used at the load points, on or about 03/21/2012. ABATEMENT OBSERVED DURING INSPECTION NO CERTIFICATION REQUIRED ABATEMENT NOTE: Among other methods, one feasible and acceptable method of abatement to correct this hazard is to operate, inspect, and maintain vehicle lifts in accordance with the American National Standard Institute - Automotive Lift Institute publication, "Safety Requirements For Operation, Inspection and Maintenance" (ANSI/ALI ALOIM: 2008) and/or the recommendations provided by the original equipment manufacturer's operations and maintenance manual.ance
