Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
On or about October 25, 2011 and prior thereto,  employees walking ,
sweeping and
shoveling rice from the floor on the 6th floor  were exposed to the hazard
of being caught by
or falling onto the moving conveyors and not being able to immediately
stop the conveyor in
the following instances:
a. the  west side of the conveyor located on the west side of the  room
b. the  east side of the conveyor located on the east side of the room
Among other methods, feasible and acceptable means to correct the hazard
include installing
an emergency pull cord or trip wire on the sides of the conveyors to stop
the
conveyor in an
emergency such as described in American Society of Mechanical Engineers
(ASME) B20.1-
2009 Section 5.11.(c).
