Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to FALL HAZARDS:
a)At Cayuga Medical Center, in the Biggs Building Parking lot, on or about
12/2/11:  An employee was exposed to a 30 foot fall while changing out
parking lot lights from a 40 foot JLG 400 CRT scissor lift.  The employee
positioned the lift on sloped ground and on an unstable surface, causing
the
lift to tip over while on the platform.  This employee was not trained on
use
of this equipment.
Among other methods, feasible and acceptable abatement methods to correct
this hazard is to:
1)  Provide training that meets the requirements of ANSI Standard A92.6-
1990 in Section 7.6.1 "Self Propelled Elevating Platforms" that states that
"the operator shall be trained either on the same model of platform or one
having operating characteristics and controls consistent with the one to be
used during actual work".
2)  Outsource the maintenance of parking lot lights to an outside company
with the skills, training, and equipment to accomplish this task safely.
ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM.
