General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards from a rapid steam expansion where a gantry crane equipped with hollow metal arms (which contained water) was dipped into a hot molten salt solution: (a)SKF USA, Incorporated, 20 Industrial Drive, Hanover, PA - Employee(s) were exposed to burn hazards from hot molten salt solution. A rapid steam expansion occurred while performing dipping operations involving a gantry crane that was equipped with hollow metal arms (containing water) and dipped into a molten salt solution resulting in a rapid steam expansion and the bursting of the hollow metal arm and the spraying of hot molten salt, thus exposing an employee to severe burns and later death, on or about September 1, 2011. Among other methods, one feasible and acceptable method to correct this hazard is to replace the hollow gantry metal arms with solid metal gantry arms, in accordance with NFPA 86 (2011), Section 13.5.15.9.1. No abatement certification or documentation required.
