Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to potential crushing injuries from a falling load while
operating a jib crane that
was not load rated and tested, inspected and marked with the load rating:
(a)Gamlet, Incorporated - Employees were exposed to potential crushing
injuries
from operating the jib crane in the BAE Area that had no load rating and
had
been installed without evaluation by a qualified engineer.  The employer
had not
conducted a load test after installation and an initial inspection was not
conducted,
on or about October 20, 2011.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to comply with ASME B30.11-2010, "Monorails and Underhung
Cranes" and
have the jib crane evaluated to determine its lifting capacity, load test
and certify the test,
inspect the jib crane and maintain the dated and signed inspection record,
and mark the
crane with its rated load capacity.
Abatement certification required within 10 days after abatement date. The
certification shall
include a statement that abatement is complete, date and method of
abatement, and states
employees and their representatives were informed of this abatement.
