Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards from utilizing defective and damaged harness
and lanyard for a
personal fall arrest system:
Establishment:  On or prior to September 21, 2011, the harness and
lanyard, installed on one
of powered industrial trucks (PIT) and used as a personal fall protection
device while performing
duties for picking orders at high elevated working levels were frayed and
damaged, and they
were neither labeled as such nor removed from service.
Among other methods, one feasible and acceptable abatement method to
correct this hazardous
condition is to:
1. Follow the manufacturer's recommendations for inspecting and replacing
harnesses and
lanyards.
2. Train employees for recognition and corrective methods before and after
each use.
3. Utilize other effective methods.
