that employees were
exposed to:

puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At shipping and receiving area of establishment of Coca Cola PR Bottlers -
the employer did not
establish, in coordination with his contractors Bestway Transport Inc and
Erasmo Collazo Rivera
Inc, a written procedure in detail specifying safety work practices.  A
driver of Bestway
Transport Inc died on December 8, 2011 when crushed by a trailer backing
up driven by a
driver of Erasmo Collazo Rivera Inc, both contractors for Coca Cola PR
Bottlers.  The drivers
were trying to disengage the rear doors of the trailer driven by  the
Bestway driver in a
coordinated operation between both drivers, the Bestway driver was beside
the trailer with the
door engaged, when the Erasmo Collazo driver back up the other trailer,
crushing him between
the two trailers.  Employees in dock and yard area are involved in
operations including
receiving, loading, and movement of trailers.  Safety issues observed
and/or documented include
the followings:
a.Limited space between trailers in dock area makes it difficult for truck
drivers
in entering
trailers in reverse, creating conditions of frequent impact to trailer's
rear door.  To
forklift operators, it creates situation of limited access for the
installation/removal of
wheel chocks.  There is also visual obstruction to verify if the wheel
chocks are in place.
b.Radio communication channel is shared among many employees (from
warehouse,
security guards, maintenance and truck drivers) creating communication
difficulties.
c.Use of forklift to disengage a trailer's rear door by impact to restore
its function,
creating crushing hazards to employees.
d. Use of a trailer truck backing up to disengage another trailer's rear
door by impacting
it to restore its function, creating crushing hazards to employees.
Among other methods, one feasible and acceptable abatement method to
correct this hazards is:
a.Provide adequate separation between trailers at dock area for truck
driver to
install/remove chocks and jacks and reduce impact to other trailers; and
also, for the
forklift operators be able to visualize and verify if the chocks are in
place before entering
trailer.
b.Establish a written procedure and safe work practices for all employees
in coordination
with other employers and contractors involved in trailer to dock
operations and enforce
those rules consistently.
c.Provide training to all employees in coordination with other employers
and contractors
employees at dock area and storage yard enforcing safe operation
procedures.
d.Assign a specific radio communication channel for employees involved in
trailer to dock
operations.e.Establish safety measures to follow when difficulties arise
as when the trailer's back door
won't close, or when others situations come up.
f.Use cones, barricades, paint or tape to designate or block off areas of
pedestrian traffic
and loading lanes.
En el area de recibo y despacho del establecimiento de Coca Cola PR
Bottlers - el patrono no
establecio, en coordinacion con sus contratistas Bestway Transport Inc y
Erasmo Collazo Rive
Inc, un procedimiento escrito en detalle que especificara las practicas de
trabajo seguras.  Un
conductor de Bestway Transport Inc murio el 8 de diciembre de 2011
aplastado por un vagon
en retroceso conducido por un conductor de Erasmo Collazo Rivera Inc,
ambos contratistas de
Coca Cola PR Bottlers.  Los conductores estaban tratando de desencajar las
puertas traseras del
vagon que conducia el conductor de Bestway en una operacion coordinada
entre ambos
conductores, el conductor de Bestway estaba junto al vagon con la puerta
encajada, cuando el
conductor de Erasmo Collazo retrocedio el otro vagon, aplastandolo entre
los dos vagones.  Los
empleados en el area de la rampa y el patio estan envueltos en operaciones
que incluyen recibo,
carga, y movimiento de vagones.  Aspectos de seguridad observados y/o
documentados incluyen
los siguientes:
a.Espacio limitado entre los vagones en el area de la rampa hace
dificultuoso a choferes
entrar los vagones en retroceso, creando condiciones de impacto frecuente
a puertas
traseras del vagon.  A los operadores de montacargas les crea la situacion
de acceso
limitado para instalar/remover los calzos.  Tambien hay obstruccion visual
para verificar
si los calzos estan en su lugar.
b.El canal de comunicacion por radio es compartido entre muchos empleados
(de almacen,
guardias de seguridad, mantenimiento y choferes de camiones) creando
dificultad en la
comunicacion.
c.Se usan montacargas para desatascar puertas traseras de los vagones
golpeandolas para
restaurar su funcion, creando riesgos de golpes y/o aplastamiento a
empleados.
d.Se usan vagones en retroceso para desatascar las puertas traseras de
otro vagon
golpeandolas para restaurar su funcion, creando riesgos de golpes y/o
aplastamiento a
empleados.
Entre otros metodos, un metodo factible y aceptable de corregir este
riesgo es:
a.Proveer separacion adecuada entre los vagones en el area de la rampa
para brindar
acceso al chofer al instalar/remover los calzos y gatos, y para reducir el
impacto a otros
vagones.  Tambien en la operacion para los operadores de montacarga, el
poder ver y
verificar si los calzos estan en su lugar antes de entrar al vagon.
b.Establecer procedimientos escritos y practicas de trabajo seguras para
todos los
empleados en coordinacion con otros patronos y contratistas que participan
en
lasoperaciones vagon a rampa y exigir el cumplir estas normas.
c.Proveer adiestramiento a todos los empleados en coordinacion con otros
patronos y
contratistas en el area de rampa y patio de almacenaje haciendo cumplir los
procedimientos de operacion segura.
d.Asignar un canal de comunicacion especifico para empleados involucrados
en operaciones
vagon a rampa.e.Establecer medidas de seguridad a seguir cuando surgan
situaciones tales como cuando
la puerta trasera del vagon no cierra, o cualquier otra.
f.Utilizar conos, barricadas, pintura, o cinta para identificar y/o
bloquear las areas de
transito de peatones.
