North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to a crushed by hazard.
a)8829 Midway West Road:  the employer did not implement a standard
procedure of utilizing
the use of the helpers platform on a Schramm (Model T450WS) Rotadrill well
drilling rig,
which creates a firm foundation for an employee to perform the job of
safely assisting the
drilling rig operator.  On December 6, 2011, an employee was injured when
trying to remove
the bushings from the drilling rig table, while standing with one foot on
the operators platform
and one foot on a diverter's PVC pipe.
b)8829 Midway West Road:  the employer did not provide safety procedures
for removing drill
table bushings without moving the drill head and chain on the mast/derrick
and
an employee
was injured on December 6, 2011, when the employee tried to remove the
bushings from the
drill table, while straddling the operators work platform and a diverter
PVC pipe, and the
employee fell into the machine and the employees' right hand contacted a
moving mast/derrick
chain.errick
