employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:

Equipment:Allis-Chalmers ACE-30-SCR (Serial Number 17209315) forklift
Condition:Employees were permitted to operate a forklift without an
installed seatbelt.
Among other methods, one feasible and acceptable method to correct this
hazard, install and utilize a restraint
system in accordance with the current version of American National
Standards Institute (ANSI) B56.1-2009,
"Safety Standard for Low Lift and High Lift Trucks," and the Industrial
Truck Standards Developing
Foundation (ITSDF) B56.1-2005, "Safety Standard for Low Lift and High Lift
Trucks."
Abatement documentation (photographs, receipts, copies of programs,
training
certification, etc.)
is required for this item.
