Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:05/02/12 Location:Parking Lot Equipment:Altec, Extensible Boom Aerial Platform Truck, Model AT37G, Serial #1200DF1983 Condition:Employees were exposed to being crushed by collapsing/failing boom since recommended inspection/maintenance schedule, as established by the manufacturer, were not followed resulting in visible problems such as structural damage, and unmarked controls. Among other methods, one feasible and acceptable method to correct this hazard is to comply with the manufacturer's instructions regarding established inspection and maintenance schedules to assure that the aerial lift equipment is maintained in safe operating condition and the American National Standard Institutes "Boom Supported Elevating Work Platforms" ANSI A 92.5-1992, Section 6 requirements entitled "Responsibilities of Owners, Testing and Inspection", Section 7; "Responsibilities of Users, Inspection and Maintenance". Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
