Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to unguarded improperly sized abrasive wheel: a)Norther Interior Maintenance Bay - Employee used an unguarded Model TL 1135 Campbell Hausfeld Air Cut-off Tool with an abrasive wheel too large for the tool, on or about 10/25/11. ABATEMENT NOTE: Among other methods, one feasible and acceptable means of abatement to correct this hazard would be to: 1. Follow the manufacturers operating instructions that state under "Operation" WARNING "Cut-off wheels must be securely fastened to the tool. Only use 3" max. diameter cut-off wheels rated at or above tool rated RPM." NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
