recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to unguarded improperly sized abrasive wheel:
a)Norther Interior Maintenance Bay - Employee used an unguarded Model
TL 1135 Campbell Hausfeld Air Cut-off Tool with an abrasive wheel too
large for the tool, on or about 10/25/11.
ABATEMENT NOTE: Among other methods, one feasible and acceptable means of
abatement
to correct this hazard would be to:
1. Follow the manufacturers operating instructions that state under
"Operation"
WARNING "Cut-off wheels must be securely fastened to the tool. Only use 3"
max.
diameter cut-off wheels rated at or above tool rated RPM."
NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE
EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION
FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN
ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR
1903.19.
