Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing, or likely to cause, death or serious physical harm to
employees in that employees
were exposed to fall hazards and electrical hazards:
On or about October 13, 2011, and at times prior thereto, employees
utilized and
operated a vehicle-mounted Elliott S70 Skywalk Hi-Reach License plate
#O62DP8
with the deficiencies listed below, the employees were exposed to fall
hazards and
electrical hazards:
a.The electrical wiring and connections did not have their protective
coverings;terminations were exposed to environmental damage.
b.Hydraulic or pneumatic systems for leaks
c.Missing or illegible operation markings of manufacture,serial number etc.
d.The personnel basket missing its top rail
e.No aerial lift maintenance record on last cable change
f.Improper winding of wire rope on sheave drum
g.Winch hook anchor missing latch
Among other methods, one feasible and acceptable abatement method to
correct
these hazard is to perform maintenance and conduct and record inspections
and
tests on the vehicle-mounted elevating and rotating aerial ladder in
accordance
with American National Standard Institute, Vehicle-Mounted Elevating and
Rotating Aerial Devices, Standard A92.2-1990, Section 8 for owners and
users,
as stipulated below:
Perform inspections of mobile units at designated intervals for the
following items:
a.Parts, such as pins, bearings, shafts, gears, rollers, locking
devices, chains, chain sprockets, wire ropes, and sheaves for wear,
cracks or distortion.
b.Electrical systems and components for deterioration or wear
including those not readily visible on a frequent inspection.
