Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which were free
from recognized
hazards that were causing or likely to cause death or serious physical
harm to his employees in
that employees were exposed to:
(a)On or about 09/30/2011 - Wells Fargo Tower, 420 20 Street North,
Birmingham,
AL, four employees cleaning exterior windows utilizing a Rope Descend
System
(RDS) were not protected from falls in that the safety line and the primary
working line were attached to the same anchorage point.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow the
guidelines of the American National Standards Institute/International
Window Clean Association,
ANSI/IWCA I-14.1-2001, Section 14.6.2 Roof Anchorages.,
